Cardiomyopathie dilatée HFrEF, FEV 30 %, NYHA classe III, LBBB (QRS 160ms). Après un traitement médicamenteux optimisé, il présentait toujours des symptômes évidents d'insuffisance cardiaque et sa tolérance à l'exercice était sévèrement limitée.
