Upper abdominal pain, weight loss 6kg, CA19 -9: 3,200 U/mL. CT showed a 4.5-cm mass in the pancreatic head and the portal vein-splenic vein junction was involved (<180 degrees). Locally advanced pancreatic ductal adenocarcinoma was diagnosed and was initially unresectable.
