MSA-P, progressive parkinsonism for 18 months with poor L-dopa response. Severe orthostatic hypotension, urinary frequency/urgency, and early cognitive decline. UMSARS-I 21, UMSARS-II 26, SCOPA-AUT 21, MoCA 23.
