Idiopathic Pulmonary Fibrosis (IPF)
Patient Condition
The patient visited the hospital due to worsening progressive dyspnea for 1 year and had a history of smoking (30 pack-years). HRCT showed extensive cellular lung. Lung function: FVC 35% predicted, DLCO 20% predicted. 6MWD 160m, mMRC score 4. Continuous oxygen therapy (3L/min) is required. Advanced IPF was diagnosed and lung transplantation was urgently evaluated after acute exacerbation.

Treatment Outcome
| before | FVC accounted for 35% of predicted value, DLCO accounted for 20% of predicted value, 6MWD 160m, mMRC score 4, requiring continuous oxygen therapy |
|---|---|
| after | 18 months after lung transplantation, FVC accounted for 80% of predicted value (45% improvement), DLCO accounted for 70% of predicted value (50% improvement), 6MWD 450m (290m improvement), oxygen therapy was removed |
| highlight | Emergency single lung transplantation was successful after acute exacerbation of IPF. Lung function recovered well 18 months after operation. Oxygen therapy was completely removed and normal life was returned. |
