Non-ischemic DCM with HFrEF, LVEF 25%, NYHA class III, LBBB (QRS 180ms), history of syncope. Non-sustained ventricular tachycardia was recorded on the 24-hour dynamic electrocardiogram, and the indication for secondary prevention by ICD was clear.
