Patient Condition
The 48-year-old patient was diagnosed with dilated cardiomyopathy and progressed to end-stage heart failure (HFrEF, LVEF 18%) after medical treatment. NYHA heart function class IV, hospitalized for repeated heart failure (6 hospitalizations in the past 12 months). The drug regimen has been optimized (ARNI+ spironolactone + carvedilol + SGLT2i) but continues to deteriorate. At the time of diagnosis, NT-proBNP was 8800 pg/mL and BNP was 2400 pg/mL. Cardiac MRI showed left ventricular end-diastolic inner diameter of 78mm, mild mitral regurgitation, and peak value VO2 in cardiopulmonary exercise test was 12 mL/kg/min.
